About this publication
This form should be used to refer a child to one of the following:
- Early Help support where you have identified a need for support to promote a child's wellbeing and the family have asked for help
- Social Work support where you are worried about the safety of a child
- Please use the South Tyneside threshold document when considering your referral
This form should be returned to frontdoor@southtyneside.gov.uk.
