All About Me form
About the form
If your child uses Home to School / College Transport please be aware that South Tyneside Council is only able to share limited information with drivers and passenger assistants. To make sure that they are aware of your child's needs, please use this form to pass on any information you feel it is important for them to know about your child.
The only information transport staff will have about your child will be what you tell them on this form.
How the form was created
South Tyneside Council have worked with Young Healthwatch South Tyneside, South Tyneside Parent Carer Forum and Your Voice Counts as well as a group of parents and carers of children with SEND to design this form.
The form was designed to be simple and easy to understand for everyone.
