Below are the steps a person follows when working with the Adult Social Care team.

Start

  • Advice, Info and Signposting from Home
  • Local Area Coordination
  • The Let's Talk Team
    • We will get in touch with you (will contact you)
    • Use of Equipment and or Assistive Technology

    • Information, Signposting and Connecting you to Communities
  • Community Based Appointments
    • Appointment with Let's Talk Local Hub
    • Use of Equipment, and or Assistive Technology
  • Care and Support
    • A conversation with you and the appropriate Adult Social Care team
    • This is about getting to know you and your outcomes, understanding your assessed needs and eligibility
    • You will have the opportunity to appeal the decision
    • We will undertake a financial assessment  
    • We will work with you to agree a care and support plan to meet your assessed needs
  • Conversation with Appropriate Team
  • Eligibility and Financial Assessment
  • Appeal Decision
  • Hospital Discharge
    • We will focus on supporting you to regain your independence and wellbeing following discharge from hospital
    • We will look at what short term support may be required to assist with this
    • Use of equipment and or Assistive technology
    • We will work with you to understand what your longer-term care and support needs are after a period of rehabilitation
    • If you need long term support, we will work with you to put these arrangements in place
    • You will have your annual review with the team
  • Allocation to Appropriate Social Work / OT Team
  • Receive Care and Support
    • Use of Equipment and or Assistive Technology
  • Annual Review with You
