Below are the steps a person follows when working with the Adult Social Care team.

- Community Based Appointments
- Appointment with Let's Talk Local Hub

- Care and Support
- A conversation with you and the appropriate Adult Social Care team
- This is about getting to know you and your outcomes, understanding your assessed needs and eligibility
- You will have the opportunity to appeal the decision
- We will undertake a financial assessment
- We will work with you to agree a care and support plan to meet your assessed needs
- Conversation with Appropriate Team
- Eligibility and Financial Assessment
- Appeal Decision
- Hospital Discharge
- We will focus on supporting you to regain your independence and wellbeing following discharge from hospital
- We will look at what short term support may be required to assist with this
- Use of equipment and or Assistive technology
- We will work with you to understand what your longer-term care and support needs are after a period of rehabilitation
- If you need long term support, we will work with you to put these arrangements in place
- You will have your annual review with the team
- Allocation to Appropriate Social Work / OT Team
- Receive Care and Support
- Use of Equipment and or Assistive Technology
- Annual Review with You
