Overview

This support, provided in self-contained and fully furnished flats in the Chichester area of South Tyneside, equips people the opportunity to gain all the necessary skills, understanding and community links to be able to live confidently and securely in their own homes and be a valued part of their neighbourhood.

Support available

The Recovery Coaching Team will help with skills such as:

  • Maintaining a tenancy
  • Staying safe at home
  • Budgeting money and paying bills
  • Applying for appropriate welfare benefits
  • Developing life skills to live independently
  • Maintaining physical and mental wellbeing
  • Accessing the local community
  • Shopping and cooking healthy meals
  • Developing a peer support network
  • Moving on to independent living

Everyone who they support through this service is linked with a key-worker who will spend time with the person receiving support, coaching them and developing a personalised support plan.

This plan will identify key areas of need raised by the person receiving support and other agencies who work with them.

This will be used to identify opportunities for the person to develop all that they need to live successfully in their own home, and is an ideal way to start the journey to recovery and independence.

This support can be provided for up to two years depending on the needs of the individual concerned. At the end of the support each person will be able to say that they can make it on their own and go forward to a positive future.

Who can access the service

People aged 18 and over who either live within or are registered with a GP within the borough of South Tyneside.

How to access the service

All referrals must be made by a health / social care professional. If you are seeking a referral, please contact your social worker, CPN, drug & alcohol worker, or other support worker to discuss your options, and they can make a referral on your behalf.
