The service aims to help people from armed forces communities feel more connected to their local area and understand the wider support on offer.
Who runs the service
Forces For Change is provided by First Contact Clinical, working with South Tyneside Council.
South Tyneside Council has received funding from the Armed Forces Covenant Fund to establish this project.
The service is funded by the NHS.

The service is free to access.
How the service can help
Forces For Change is a scheme which helps connect ex-service veterans with the armed forces community and highlight the services available to them.
We aim to help people from armed forces communities feel more connected to their local area and understand the wider support on offer.
There is already excellent support in South Tyneside for the armed forces community and we'd like to help link you in with this existing support.
First Contact Clinical will offer coaching to individuals who have been identified as needing additional support.
The scheme aims to:
- Identify what is affecting an individual's wellbeing and how they would like things to be different.
- Build knowledge, motivation and confidence to make changes.
- Set and achieve personal goals to improve wellbeing.
- Connect with people, activities, groups and opportunities in the area.
- Develop strategies for managing mental and/or physical health conditions.
- Navigate/coordinate your support if multiple services are involved.
Location
We have a team in every GP surgery in South Tyneside.
Your appointment frequency and length are decided by you.
